A) shortness of breath on and off for the past 20 years, her latest episode of shortness of breath started 30 days ago, her SOB was insidious in onset and gradual in progression.  Pedal edema since 15 days upto the level ankle and pitting type. Facial puffiness since 15 days. She has drowsiness since 2 days She has decreased urine output for the past 2 days. Anatomical localisation :occupational cause Due to continuous inhalation of paddy dust, affecting lung parenchyma  ETIOLOGY AGENT: paddy dust act as a triggering factor Paddy dust is biologically composed of  plant material fungi: of genus epicocum, fusarium parts of insects: House dust mites and storage mites bacteria soil Rice dust enters hum
